By using the Clarity Dental Collective website or booking with us, you agree to these terms. If you do not agree, please do not use the site.

Services Provided

Clarity Dental Collective provides general dental care, including exams and cleanings, fillings and crowns, cosmetic dentistry, clear aligners, extractions and implants. All care is provided by licensed dentists in line with the law that applies to them.

Appointments and Cancellations

We ask for 24 hours' notice to cancel or change an appointment. Late changes or missed appointments may be subject to a fee, which we explain when you book. We may reschedule an appointment when we need to for clinical or practical reasons.

Patient Responsibilities

Please give us accurate health information, tell us about changes to your health or your medicines, follow the care instructions you are given and let us know if you have concerns.

Fees and Payment

How fees work, including insurance, paying without it and written estimates, is explained on our How Fees Work page. Your dental plan decides what it pays; you are responsible for any amount it does not.

Limitation of Liability

We provide dental care with reasonable skill and care. Results vary with each patient's circumstances and with how closely care instructions are followed, and some factors are beyond our control.
